POSITION DESCRIPTION: The Medical Records Clerk will support the Patient Administration Systems and Biostatistics Activity (PASBA) processing center on Fort Sam Houston, TX. Duties will encompass record processing and scanning, of miscellaneous outpatient records/loose documents to include long term storage. Specific duties will include but not limited to: Maintain medical records in accordance with (IAW) appropriate Army Regulations and Deployment Medical Record Processing Center (DMRPC) User Guides. Identify the unit and inventory records received. Check records against electronic listing and document record data on required reports. Notify supervisor or lead of receipt of boxes, unit, record counts and missing records Separate records by patient category and file in terminal digit order by active duty, detainee and all other patient categories and re-box records if necessary. Write the box number and unit on each record folder. Work all active duty and detainee records first. All other patient categories records will be worked when all active duty and detainee record processing tasks are completed. Print box label with facility name, unit identifier, location of unit, DMISID, reporting MTF code, event code, unit zip code, and date received. Print separate label with box number and place both labels on box. Create an inventory listing for all records in each box to include MTF/DMISID, split unit identifier, received date, register number, SSN, Patient Name, Patient Category, Box number, Identify if Disease(D) or Injury(I) and place listing in front of box. Assign and attach Iron Mountain tracking numbers to boxes and transmittal sheets. Maintain copies of transmittal sheets. Check and verify listing in boxes and assemble records IAW appropriate Army Regulations and Deployment Medical Record Processing Center (DMRPC) User Guides. Remove all staples in record and separate and tape individual lab sheets. Notify supervisor or lead of missing records. Collect, audit, research and print information from internal and external databases to ensure proper data elements are abstracted into all applicable patient administrative applications and databases. Accurately enter demographic and clinical information into all applicable patient administrative applications and databases using a personal computer. Print the DA-3647 and insert into record. Print box listings and labels in patient administrative applications and databases. Verify name, SSN and register numbers on box listing and labels for accuracy and legibility. Make corrections accordingly and attach labels to records and insert box listing into box. Notify supervisor or lead of missing and duplicate records Run record data discrepancy reports in all patient administrative applications and databases. Identify and correct data discrepancies in all applicable patient administrative applications and databases to include copy and rescan of corrected record data. Insert and/or replace corrected pages into medical record. Reprint label and correct folder if applicable. Assist in preparation of deliverables as deemed necessary by the medical records clerk lead. Verify box listing with records in box prior to copying. Notify supervisor or lead of missing records Copy both sides of all medical record documents Abstract record data to the DA-2985 and insert record. Verify data IAW appropriate Army Regulations and Deployment Medical Record Processing Center (DMRPRC) User Guides to insure all data is accurate Scan copied medical records into appropriate patient administrative applications and databases. Run scan record tracking list and verify against box listing in box. Assist in training other medical records clerks as deemed necessary by the lead medical records clerk. Assist in flagging records, incrementing versions, processing records to the Standard Inpatient Data Record, correct failed records in all applicable patien
