Job DescriptionDESCRIPTION OF DUTIESThe primary goal of the Eligibility Specialist is to screen all clients for program services and register clients into the county CPCDMS system.CORE FUNCTIONSActivities of a Eligibility Specialist will include, but is not limited to the following:Identifying and screening clients for program servicesIdentifying and screening clients for program services (administers all registration and intake processes for the various programs)Ensures eligible clients are enrolled for the appropriate servicesComplete Harris County CPCDMS registration dailyConduct on-site registrations at primary care sites (ie, SHF-Conroe, SHF-Bellaire, SHF-Stafford, etc.) and other community based organizations where collaborations are establishedResponsible for providing in-services to community based organizations and clinics on accessing agency programsVerifies client Medicare/Medicaid/Private Insurance eligibility and documents within the clients recordsAssist with preparation of client records and retention of client recordsAdvocating on behalf on the client to decrease service gaps and remove barriers to servicesData entry into Access, ARIES and CPCDMS SystemsSKILLS /TRAININGS Must have prior experience in performing Medicare/Medicaid and insurance billing and verification; Computer proficient, excellent written and verbal skills. EDUCATION /EXPERIENCE HS Diploma, and at least one year paid work experience working with persons with HIV/AIDS or two years documented volunteer experience.Company DescriptionSt. Hope Foundation is a nonprofit 501(c)(3) community healthcare organization that was established in November 1999.
