Registered Nurse (RN) Patient Care Navigator

Kingwood Medical Center is currently accepting applications for a Registered Nurse (RN) Patient Care Navigator. This is a full time/days position. .
Serve as the day-to-day lead of Kingwood Medical Center’s Orthopedic Service Line (OSL) to develop, implement, and organize the program. The Orthopedic Care Coordinator (OCC) will coordinate the efforts between surgeons, surgeon’s offices, hospital directors and other staff while tracking the progress of the program and identifying areas requiring attention. The OCC is accountable for all administration, report generation and review, process improvement, patient satisfaction, facilities review, budgeting (if required) and activities of the program.
Major Tasks:
  • Coordinate regularly scheduled team meetings with hospital administrative, medical, clinical, and other key staff.
  • Monitor the build out and oversee the day-to-day operations of the OSL program.
  • Coordinate in the review, revision, and development of standardized care plans with the surgeons including: pre-op, post-op and discharge orders.
  • Ensure appropriate pre-op preparation and coordination for each OSL patient prior to surgery.
  • Develop and oversee the creation of pre and post-op teaching classes and educational material presentation, including the coordination of teaching materials with the physicians’ office.
  • Create a measurement system for tracking surgical outcomes and patient satisfaction, and record data for identification of areas that require improvement.
  • Provide key statistics at Leadership Team meetings and to other stakeholders as necessary. Statistics may include patient satisfaction and outcomes, progress against budget, complications, denied days, hospital-wide survey results, nurse call-back results, etc.
    Patient Satisfaction Duties:
  • Perform follow up calls of patients for satisfaction reports
  • Develop a program for community education through luncheons and/or seminars inclusive of schedules, presenters, content and materials
  • Supervise the continued improvement and revision of unit-specific practices and patient education materials to include Nurse/Coach Discharge Checklists, Patient Manuals, Daily Bulletins, Wall Educational Posters, and other standardized resources for the Orthopedic Service Line.
  • Create and direct an ongoing quality improvement plan
  • Ascertain patient services and secure resources to administer the services
    Will be responsible for knowledge and skill necessary to provide care based on physical, psychological, educational, safety and related criteria for the adolescent, adult and geriatric population.
